[xdebug-general] Executed lines missing from coverage

Looking at the above image you can see that lines 10, 11, 16, 23, 30, etc are not reported by xdebug as being executed. However, it's clear they are.

Additionally in that image is a print_r of the array returned by xdebug. It shows the missing lines in the array key values.

Am I missing something? (or is xdebug missing something?)
