[xdebug-general] Getting "Error Code = 3" in response to every command

My installation went fine, and my phpinfo() looks great, but when the
server connets to the client, I get an error code of "3" in response to
every command. What is error code "3"? I haven't been able to find any
info on error codes. I've included a sample session below. Any help
would be greatly appreciated.
