Yeah, the tailored instructions seem to be wrong. Xdebug 2.1.x doesn't
support PHP 5.4. You'd want the Xdebug 2.2.0RC2 release:

http://xdebug.org/files/php_xdebug-2.2.0RC2-5.4-vc9.dll

I'll fix the tailored instructions.
